As stated in Barkley, a project functions a system which utilizes a series of tasks to produce a new product (2014). This new project can be a new physical product or a new process of which operations will be conducted. Therefore, it is temporary and unique in nature. The outcome would depend on the goal of the stakeholders (Barkley, 2014).
A few years back, the hospital that I worked at introduced the Transformation Care model in standardizing work flow and improve efficiency. I was part of a team tasked with decreasing readmission rate. We completed the cycle of project management in order to come up with a new discharge process and education materials to reach the goal of lowering readmission rate.
Initiation: the project was selected because of Obamacare, any patient readmitted to the hospital within 30 days of their discharge, regardless of their mental health or compliance with their treatments, the hospital would not get pay for their subsequent hospitalization for the same diagnosis.
Planning: we identified issues such as not enough time to go over patient teaching at discharge; teaching materials were too complicated and only in 2 languages and patients needed help navigating the clinics and appointments.
Execution: We introduce large print and colorful materials in 5 different languages to match the needs of our patient population and new work flow of start discharge teaching when the patients were admitted. This allowed time for them to understand and ask questions prior to discharge. Appointments were made for patients who needed help.
Monitoring: This project was rolled out in one nursing unit and resource person was on hand to educate and oversee work flow and make necessary adjustments.
Closeout: The project produced a new work flow and new teaching materials. These new products helped decrease readmission rates when metrics were analyzed. These products are still being used to this day throughout that hospital.
The Transformational Care team was a temporary group of representatives from all the stakeholder departments and we came up with a new product at a given time frame which met the needs of the stakeholders. We then moved on to working with other improvements. Therefore, it was a project and not an operation.

Describe a project you were on. Decide if it was a project or part of ongoing operations.
One project I was part of was the development of a new Security Operations Center (SOC) for a customer in Silicon Valley. Three of us went to a customer site as part of a three-month project to create the new SOC. The customer had an experienced manager to lead the project. She gathered a project team, which is a characteristic of project management (Barkley, 2014). In addition to contracting us to help for three months, she had a skilled cyber analyst and a junior team member with suitable skills. She was also actively looking to hire more staff. The goal was to build a functioning SOC in three months. By the end of the project, the SOC would have a permanent team of trained cyber analysts with continuous monitoring in place. The SOC would have processes and quantifiable metrics to show progress. Upon completion of the project, the SOC would be put into production and become operational, and we would go back to our primary jobs.
The project manager had exceptional interpersonal skills with extensive cybersecurity training. She was flexible but made sure that we understood the mission. She managed schedules, the project budget, and the quantifiable outcome of our efforts. According to Millhollan and Kaarst-Brown (2016), these metrics enable a project manager to illustrate task completion and success or failure. Within three months, we caught an active insider threat attempting to hack servers. As a project requirement that was noted in the lecture (L. Alnaji, personal communication, January 2020), this project was a temporary, one-time endeavor, with a clearly defined start and end date. There was no scope creep or deviation from the mission, and it was a success.

A recent project I was involved in was to deliver an automated results dashboard. I had originally built the dashboard using Excel and would have to manually pull the data from multiple sources, input the data, and refresh the dashboard each month.
The first phase is to align and kick off the project. One of the activities I was a part of for this project during the alignment and kick-off phase was defining the scope and the business case (Barkley, 2014, sec. 2.2). During this activity we defined the scope to be a duplication of the Excel dashboard I had already created, however, the data pulls would be automated and the presentation of the data would be in a different visualization tool called Tableau.
The second phase is the planning phase During this phase a plan is laid out for how the project will be managed as well as the plan to tackle the actual project request (Barkley, 2014, sec. 2.4). One of the actives I was part of during this phase was discussing the project requirements. During this activity I met with the project team to discuss the plan and what requirements I need from the final product.
The third phase is execution. During this phase the work is carried out. The fourth phase is monitoring and corrective action, which means to adjust as issues or problems some up. During these phase, one of the actives is to report on the project progress or any issues (Barkley, 2014, sec. 2.5). We had monthly meetings with the project team to talk about the current progress and lay out additional expectations as things changed.
The fifth phase is the project closure and assessment. During this phase the project is achieved and the customer (me) signs off on the work. The activity I was involved in during this phase was to review and sign off on the finished product.A phase-gate review is a desirable requirement in any project because it can increase profitability, keeps the focus strategic, reduces errors, and promotes communication and collaboration (Smartsheet, 2020). The phase-gate approach builds a structured approach to the project from initiation to closure and insures a successful launch.

We also determined the cost and time frame it would take to complete. Phase 3 (Execution): We introduce large print and colorful materials in 5 different languages to match the needs of our patient population and new work flow of start discharge teaching when the patients were admitted. This allowed time for them to understand and ask questions prior to discharge. Appointments were made for patients who needed help. Phase 4 (Monitoring): This project was rolled out in one nursing unit and resource person was on hand to educate and oversee work flow and make necessary adjustments. Auditing were done regularly. Phase 5 (Closeout): The project produced a new work flow and new teaching materials. These new products helped decrease readmission rates when metrics were analyzed. The new work flow was introduced throughout that hospital.
Phase-gates are hard stop reviews that was conducted at the end of each phase to evaluate if the project was on track and identify unforeseen issues. Just like the monitoring phase, it's done throughout the project. The scope of the phase-gate could be different in each phase. If the project or cost was identified to be no longer align with the organization objective, the project can be stopped. (Barkley, 2014).
