Deliverable 4 - Population Health Management Dashboard
Population Health Management Dashboard
Deliverable #4
Dr. M. Point
Presentation goals
To describe the requirements for deliverable four
To examine the role of dashboards in PHM
To analyze the deliverable requirements
To discuss best practices for submissions
To review evaluation determinants for submissions
Motivational quote
Welcome to module four
For this module, you will gain the competency of applying data analytic methodologies to diverse populations to address population health needs
Connecting the dots
Module One
Describe a local healthcare profile (population demographics and statistics)
Smoking Cessation Program
Assess populations health needs using the CHNA
CHNA indicates large number of residents are diagnosed with Lung Cancer and #s are growing
Identify KPIs
# of residents enrolled in program; # of residents who quit smoking through the program, …..
Connecting the dots
Module Two
Preparing an Executive Summary
Identify data sets needed to support the company’s PHM
# of individuals who smoke; # of individuals who have parents who smoke; average # of years individuals smoke, etc.
Module Three
Identify sources of data
What sources and types of data will you use?
ICD-10CM; Data from the EHR; Survey responses
Discuss the use of readily available data
Creating data dashboards
A data dashboard is an information management tool that visually tracks, analyzes and displays key performance indicators or KPI, metrics and key data points to monitor the health of a business, department or specific process (Carson, 2010).
The 1900’s
No speedometer
Max speeds of 35 MPH
Car dashboards
As cars evolved, the issue of safety gained importance and in 1948 the Tucker became the first car with a padded dashboard. By the 1970s padded dashboards were the norm (Fears, 2017)
So, how has the dashboard evolved?
Data Dashboards and Car Dashboards
Think of a data dashboard like a car dashboard—but instead of showing your speed and fuel level, it shows things like hospital wait times, infection rates, patient satisfaction, or financial performance (in general healthcare).
For your population management programs, it may show things like – average age of individuals living in a particular community; number of teens who report having unprotected sex; average income of residents in a particular community
Big data
The more data we have available, the greater the detail in dashboards
Questions
Scenario
You have assessed your local population health needs and identified data sources and data sets that are need to help providers make immediate gains in patient outcomes. Your health systems Board of Directors is requesting that you develop a high-level population management dashboard.
Scenario
For this assessment, you need to assess local population health needs and identify data sources and data sets that are needed to help providers make immediate gains in patient outcome. It is a major undertaking to plan, design, and implement a robust PHM.
Therefore, your health systems Board of Directors is requesting that you develop a 1 - 2 page high-level population management program dashboard.
Scenario
In this dashboard, list the health needs based on the community needs assessment and the critical data sources and data sets needed for the population health management program your health system is planning to launch.
For example, teen pregnancy prevention program – what data did you identify as necessary to make the decision to implement your program?
Refer to the data from previous deliverables (review the data from Deliverable 2)
Instructions
Using the information from the modules 1, 2, and 3 summative assessments, construct a dashboard that lists that health needs based on the community needs assessment that was performed and the critical data sources and data sets needed for the population health management program your health system is planning to launch.

Example
Teen Pregnancy Prevention Program
Data sets – Number of teens in a community; income levels, education levels, age, sexual status
Create a visual – bar graph; pie chart; line graph
BAR GRAPHS
A bar chart, also known as a horizontal column chart, is popular for a reason — it’s easy on the eyes and quickly visualizes data sets.
With bar charts, you can quickly identify which bar is the highest or the lowest, including the incremental differences between bars.
PIE GRAPH TEEN PREGNANCY
A pie chart represents numbers in percentages, and the total sum of all pies should equal 100 percent.
When considering charts and graphs to employ to visualize data, pie charts are most impactful to your audience if you have a small data set.
LINE CHARTS
When to use line charts
Compare and present lots of data at once.
Show trends or progress over time.
Data Dashboard Example
Number of Teen births per 1000 by race
The trends of Teen births in the county
Outcomes of Teen births ages 15 – 19
