SAMPLE FBA/BIP ASSIGNMENT

Johnny is a small-for-his-age, 10-year-old Hispanic boy. He is having significant problems doing his work in class because he is easily distracted by noise around him, gets frustrated easily, and does not relate well at all to his peers. His parents have not spent much, if any, time at school; and they do not attend school activities. Little is known about his family other than that there are many siblings at home, he shares a bed with several siblings, and mother is a single parent. Johnny is often teased by his peers. When the teacher left the classroom unsupervised one day, Johnny was teased by a classmate. Johnny became upset and loudly threw his books on the floor. The rest of the class immediately became quiet and left Johnny alone.

A. Family Factors: Hispanic, many children at home, single-parent mother, not involved in school

B. Personal Factors: Small for his age, poor social skills, easily frustrated

C. School Factors: Teacher left classroom without any adult supervision

D. Antecedent: Teasing by peer

E. Problem Behavior/Target Behavior: Threw his books on the floor when teased

F. Function of the Behavior (maintaining consequences): Teasing stopped

2.BIP

A. Target Behavior: Throwing books on the floor when teased

B. Strategies to prevent Target Behavior:

1. Teacher should not leave room unsupervised – she should be counseled by principal within 1 week of the date of this document.

2. Social climate in classroom needs to be addressed. Social worker to make at least one presentation to entire class about teasing and bullying within one week of incident. And, if students who were teasing can be identified, they should be given 2 additional individual counseling sessions by social worker within 2 weeks of incident. Social worker to report results of these interventions to the school counselor within 3 weeks of incident.

3. Student needs training in coping skills, and anger management – training to be provided by school counselor in 12 sessions conducted twice weekly, starting within a week of this document’s date. To be completed and reported by school counselor 6 weeks from date of this document.

4. Student needs training in social skills – training to be provided by social worker in 8 weeks of social skills group meetings. Group to start within 2 weeks of the date of this document and meet once per week. Social Worker to report progress and completion to school counselor 10 weeks from the date of this document.

5. Student may benefit from having a different seating arrangement in class that might reduce the likelihood of teasing and/or decrease the student’s problem with distractibility in the classroom. This can be discussed with teacher by principal and mentor teacher within a week of the incident, all of whom will help classroom teacher identify 3 alternate seating arrangements. Results to be reported by teacher to school counselor 2 weeks from date of this document.

6. Classroom teacher and counselor to begin forming relationship with mother and provide support – weekly home visits should start within 2 weeks of date of this document for at least 6 weeks. School counselor has responsibility for overseeing completion of these interventions.

7. Student in need of psychological evaluation to determine the likelihood of ADHD. School counselor to make referral to school psychologist within one week of the incident. Because it can take some time for the psychological evaluation to be scheduled, school counselor to follow up on status of evaluation every 2 weeks until evaluation is completed.

C. Strategies to Deal with Problem Behavior (if/when it recurs):

1. Check to see that all interventions listed above have been conducted or are being conducted and determine if more or different intervention is needed.

2. Do another FBA to see if more information can be gathered on the behavior and determine if prevention steps above were beneficial and/or need to be changed.

3. When behavior reoccurs, calmly remove child from situation. Address the behavior of any peers who seem to have provoked or contributed to the unwanted behavior.
