What will be defining factors of success or failure in these models? Use your textbooks as a starting point for your research and supplement this search with research to find relevant content.
While preparing your report, make sure you include information on the following:
- Demonstration projects related to ACOs or PCMHs.
- Differences from our current system of care.
- Risks to providers of care in these models.
- How government and payers of health care are attempting to get providers to adopt these models (incentives?).
