Visual Basic 2010 Modified Shipping Hub App

I need help creating an app in Visual Basic.  A zip file and picture of the instructions are included.The app I am trying to complete is from Simply Visual Basic 2010 (Dietel) exercise 20.11 (pg. 476).

Also, steps J) and K) are on the next page so I will include them here:

     J) RUNNING THE APP- select "Debug > Start Debugging" to run your app.  Add several packages.  It the "Packages by Destination" GroupBox, select a state for which there are packages being sent.  Doiuble click one of the packages listed in the "Packages by Destination" ListBox, and verify the correct information is displayed in a MessageBox.

     K) CLOSE THE APP
