Summarized Cash Flow and Proforma for 6 months

I have the rest of the business plan done and will attach it, but need help with the numbers side of it:

 

Your final business plan should have the following financial (simplified) statements (based on your reasonable assumptions of revenues and expenses):  a cash-flow statement for at least six months; and a proforma (estimated) income statement for six months showing revenues and expenses and profitability (or losses).

 

It should be 1-2 pages for each, with made up projected costs for a small start-up boutique. The only numbers I have are $4,000 a month in rent, and $10.00 an hour for 5 employees.
