The goals and objectives determined in the designing stage of a program can be used as standards to which the outcomes of the program can be compared.  The more specific the goals and objectives, the easier it is to evaluate the effectiveness of the program.  Refer to the CDCynergy Guide Phase 5: Plan Evaluation.  Your assignment for this week must contain each of the following sections:

  1. Summarize the process of program evaluation and include the “universal” components that are found in most evaluation models. 
  2. Develop an evaluation design

    1. Which sources of information and methods will be used to collect data? 
    2. What methods will be used to compare program outcomes to program goals? 
  3. CHILD OBESITY is the program.  Determine the data that would be collected before, during, and after the program. 
  4. Apply a data collection method to the program design.

    1. Explain why you selected this method. 
    2. Describe how the method will be used. 
  5. Contrast the outcome of the projected goals. 
  6. Appraise the value of evaluation. 

Your assignment must be a minimum of four-pages in length (excluding title and reference pages) in APA format, and should include a minimum of four scholarly sources cited according to APA format.
