Instructions:

1.     Develop a worksite wellness program using the “Worksite Wellness Toolkit” located in the Resources folder in the  Online course site.

2.     Options for your worksite wellness program:

a.     Approach an existing business and offer to develop a worksite wellness program for them.

b.     Develop a worksite wellness program using an existing business as a model.

c.     Develop a worksite wellness program for a fictitious business.

3.     Produce a document explaining and outlining your program.  Document must contain the following:

a.     Title Page

b.     Company Information and Background

c.     Step 1: Building the Foundation

d.     Step 2: Assessing the Worksite

e.     Step 3: Creating a Wellness Plan

f.      Step 4: Implementing the Plan

g.     Step 5: Evaluating the Program

h.     Conclusion

i.      Reference Page

4.     Produce a presentation of your program (PPT, Video…be creative)

Present your worksite wellness program to the class.

NOTE: I attached the Worksite Wellness toolkit.
