Part 4 Research Utilization Implementation Plan

Continue working with topic of Collaborative Teams - Complete the Health Care Change Project Matrix in the Materials section of the classroom using below criteria:

Describe the methods to be used to implement the proposed solution as follows:

·     Develop a detailed plan for implementing the proposed solution.

·     Incorporate a change theory to develop the implementation plan and explain how it is used to develop the plan. 

·     Identify resources needed for the proposed solution’s implementation and how you plan to gather and incorporate them. 

·     Discuss the feasibility of the implementation plan.

Format your matrix consistent with APA guidelines. Include a minimum of 4 scholarly references.  Include an APA formatted title and reference page.

Submit the assignment via the Assignment Files tab.
