NUTRITION PLAN ASSESSMENT

Look I need a case study as if it was a real patient for this class. I want a teacher who can get this done within the next few hours and no plagarism!! My teacher keeps sending me back with stuff that is not accurate. I need a teacher who has experience in Nutrition or atleast understand the concept of what I need. I am uploading the document of what it is she is looking for and I need all references!!! I will have her look over it before I submit it so please don't waste my time because one teacher has already done it!! It is due tomorrow and I need it done today or in a few hours to submit to her for approval. THANKS
