You will need to fill out and turn in the following forms and schedules (and put them in this order): Form 1040, 28% Rate Gain Worksheet (page 10 of the Schedule D Instructions), Schedule D Tax Worksheet (page 13 of the Schedule D Instructions), Schedules A, B, C, Forms 4562, 8829, Schedule SE, Forms 8949, 4797, and Schedules D and E.
