For this survey, do a quantitative Gratifications Study (ex. Internet use) or a quantitative Effects Study (ex. effects of videogame violence). 

1) Develop 4 survey questions related to your study. 
Use the "Never, Rarely, Sometimes, Often, and Always" scale for every question. 
Give examples for each question.
(See my sample survey questions below.)

2) Give your survey to 5 people in person or online. (This is your sample group.)

3) 
Find the average of each question; see below for sample calculations. 

4) Results Analysis: Discuss your results based on the average for each question.

Because this is a 
quantitative research project, you must be able to quantify your variables and results. Do not ask open-ended questions. (See my two sample questions below as examples.) Never, Rarely, Sometimes, Often, and Always can be given numerical values such as Never="0," Rarely="1," Sometimes="2," etc. Use the "Never, Rarely, Sometimes, Often, and Always" paradigm for every question you ask.

How to do the above calculations...
It is simple multiplication. For question #1 about email, 1 person said Often and 4 people said Always.

Often is given a weight of 3 and Always is given a weight of 4.

So you would multiply 1X3 and 4x4. That is 3+16 then divide by the sample group # which is 5 for this survey.

That means your average for question #1 is 
3.8 (...that is very close to 4...and 4 means Always).
