Establish causation and discuss the effects of a work-related incident. Conclude with recommendations to prevent a recurrence
The focus of Assignment should be directly related to a workplace incident one might expect to encounter in your chosen profession. Pages 347-350 of the textbook detail the steps to follow for a complete incident report. You'll follow a limited version the same basic steps, because for this writing task you're not expected to write a report of comparable length to the example on pages 348-349.
Use a memo written in MS Word attached to a Bb email with Assignment in the subject line. Follow this format:
Subject line, which will "identify the precise problem or event and the date it occured."
Opening, which "provides a brief summary statement noting the incident/accident, the date it took place, who it primarily affected, and what the result was."
Details, which "gives a ... description of the problem - what happened, what were the contributing factors and what was the root cause of the event -- note: here you need to be sure to establish causation, i.e. make it clear your observations are based on fact as opposed to speculation.
Aim for 300 words. You can use graphic highlighting within your memo such as bullet points, italics, bold, and underline if/when you feel it's helpful.
