I need someone put the attached ERD digram in SQL or Access tables

I need some one to do this for me I already did the ERD I just want to put it in tables either SQL or Access, and follow this instractions: 

     You will need to implement this design in MS Access or MySQL.  

    You will need to define a minimum of:

a.      5 queries on the data

b.      3 forms for entering and retrieving data

c.      3 reports that can be generated from the data
