I need a full case study / workup on male GU issue

I have the template that needs to be used. No specifics, you can make up any kind of patient info, as long as it involves a testicular, male GU problem, with differential diagnosis, diagnosis and secondary and tertiary interventions as well as complementary/alternative therapies. I am also providing the gradic rubric which will be used for this assignment.
