I need a flow chart for business communication gone wrong and fixed ~ any scenario will work

Dear professor,

I am currently trying to understand the communication process in the business world. I am expected to write a short scenario in which the communication process does not go well for the people involved. Manager directs orders not understandable to all readers, etc.

Then I need to map out the process to show where communication broke down using labels in the process example to show who the people are, what the barriers are, what contexts exist, what messages were encoded and decoded, etc.
I aspire to earn my degree in business mgmt. but right now I'm overwhelmed!

I know I need to explain how the communication process could be improved and then draw an illustration of how the improved communication process would work. It doesn't matter which medium I use (Power point, Word's shapes).
