Help with Microsoft Excel 2013 Assignment!!!

Hello. I need help with and Excel 2013 assignemnt.

The pages that I need completed are on the first document entitled assignment_project_4g_and_4f.

The other attachements are the documents that need to be used to complete the assignemnt. The 1st attachement will tell you which form to use. 

I need it completed by 10:00pm. pacific time. 

(note: you do not need to insert a header or footer)
