Fix this microsoft access submission

Need help editing this assignment. I have attached the file that needs to be editted. I've also attached screenshots of the assignment page with the instructions given to me. Just needs minor fixes as listed below:

-missing phone input mask

-default values for city and state

-remove quotes from beginning and end of date format

-add a new calculated field for units * cost. The currency one there now cannot be changed. delete it when you have your new one or delete it first and re-make it.
