Evaluation and Management Services are the most common codes used in a physician’s office. The three key components that drive the selection process are:
History
Examination
Medical decision making

Review the material in your text including the Tables 7.2, 7.3, 7.4, and 7.5 to fully describe the elements required and differentiate between the elements.
