In addition to the patient's diagnosis, the other pieces of information associated with every health care encounter are the procedure codes, the dates of service, the location in which they were performed, and the physician or other provider who performed them. Topic - Procedure Coding - Answer each of the questions:  1. How does the location of where a service is performed determine which code set is used. Relate issues such as reimbursement schedules, co-pay amounts, and coverage limitations. 2. What is a fee schedule? 3. What is a copay? 4. What types of services does the term "procedure coding" include?
