Disclaimer for a System Design

I am not a good at writing and I need to write a disclaimer to go the screens for a prototype I am designing for a web application.  Can someone help me write the disclaimer.  The disclaimer should state the web application is in a proof-of-concept / non-production state 

and line  or time stamp stating the date of the oldest data feed.  If you could provide at least 3 or more examples of disclaimers.   Due No later than 7:00 p.m. tonight.  Thanks.
