Personal Health Records (75 Points)

You are a health information management professional who has been asked to make a presentation to a community group on the topic of personal health records. Discuss how you would structure and present this topic to the community group.  Include the topics of how the concept of a personal health record developed, why a patient would want to create and maintain a personal health record, and what that record should contain.

Your report should be 2-3 pages long (excluding the cover page and reference page), supported by the readings from Module 3 and at least two additional scholarly sources.
