1. Research three new companies that sell the CMS-1500 claim form.

2. Compare costs and determine the least expensive place to order the form and note shipping schedule if available.

3. Write up a brief summary (150-200 words) with your findings of which companies you researched. Create a cost analysis and write up a recommendation for your office with comparisons of this information. Based on the research you found, which company would you recommend the practice purchase from going forward and why?
