Since one of your data collection instruments appears to have Likert scale style responses (i.e. rate on a scale from 1 to 5) it appears that your methodology is really mixed methods because you are using both qualitative approaches (open ended survey questions) and quantitative approaches (numerical types of responses). Please clarify within the methodology section.
