For each community resource you identified, do the following: Explain why you believe it to be an appropriate referral for Mr. Moore specifically. Discuss what you believe Mr. Moore will get out of the referral to this resource. State your goal for Mr. Moore in terms of referring Mr. Moore to this resource. Rank each of the 3 resources to which you referred Mr. Moore in order of most needed to least needed.
