3- On page 183 of the text is a commentary on quality and ethics. Pick a recent quality issue that has been in the news and had fatal consequences and discuss the role of Quality Management and how known quality problems are missed, improperly or not reported. Elaborate.

For example the space shuttle Challenger disaster was a result of an o-ring failure due to cold weather that was flagged as an issue but not listened to by management due to a need to fly the mission - safety took a back seat.

4- The goal of SPC is to control variation in a process. Discuss what variation means and what the sources of it are? Can all variation be eliminated? How can a process go out of control?
