Part 2 of Change Plan: Using Lewin’s Change Theory to Develop a Practice Change Plan 

Based on your assessment of the issue at the facility that you identified in XC4001, propose a practice change to address one issue related to population health or quality and safety at your practicum site. The scope of the change should be limited to actions nurses can take;  it should be a change that is under nurses’ control. Your change should be based on Lewin’s change management theory. 

Use the  XC4002 Module 2 Pre-Assessment Performance Task Submission Template to complete the Assessment.  

Part 2 of Change Plan: Using Lewin’s Change Theory to Develop a Practice Change Plan

Based on your assessment of the issue at the facility that you identified in Module 1, propose a practice change to address one issue related to population health or quality and safety at your practicum site. Module 2 Pre-Assessment Performance Task Submission Template

Using Lewin’s Change Theory to Develop a Practice Change Plan

1. Unfreeze: Summarize Lewin’s “unfreeze” phase and explain it in relation to your practice problem and corresponding change plan (2–3 paragraphs).

a. Look over the evidence that supports the change or similar interventions (use at least 3 scholarly sources).

b. Identify the stakeholders who would need to be informed of or involved with the change.

c. Explain who would be impacted by the change and how.

d. Describe the driving and restraining forces behind this change.

2. Change: Summarize Lewin’s “Change” phase and explain it in relation to your practice change (2–4 paragraphs).

a. Provide an overview of the change plan.

b. Outline the steps or phases of the change plan. Include a timeline for various steps/phases (depending on your change plan, the time-to-completion may vary and does not have to be limited to the length of the course).

c. How will you garner support for the change? What kind of support will you need? Who will you communicate with to find the support?

d. Identify resources needed to implement the change (i.e. funding for staffing).

e. What training will you need to provide, and to whom?

i. Note: You must implement the educational component of the plan yourself, between Weeks 4 and 5 of the course.

3. Refreeze: Explain the “refreeze” component of your change plan (1 paragraph).

a. How will you evaluate the effectiveness of the change?

b. How will you sustain or reinforce the change?

Part 2 of Change Plan: Using Lewin’s Change Theory to Develop a Change Plan

1. Summarize the practice problem (1 paragraph). Explain the practice problem from Part 1. You may use components from Part 1 of your Assignment (in Module 1) in this section. If your instructor has provided feedback and suggestions for revision of your practice problem, implement those changes before summarizing the practice problem.

2. Propose a change plan that addresses the practice problem (1 paragraph). Explain the organizational change you propose to address the practice problem you have identified (i.e. adopting a new technology, revising a care protocol, implementing standardized procedures, or introducing innovative patient-centered processes). Explain how this change would improve population health or quality and safety.
