Create a simple form on your webpage.  The form needs to include   1. At least 3 text input fields, either radio buttons or checkboxes (or both) (10 points)  2. A password field (related to a user id field) (15 points)  3. A text area for comments (10 points)   Make sure all input types are unique, based on the behaviour  The form controls should make sense as a collection of controls and load in browser as expected. (10 points)      Also, after the form, Create a table that contains images in one of the columns related to data in the table.   Note, you will have to deal with the size of images. (20 points for creating a table and 25 points for embedding an image)     10 points for alignment, code logic, structure and the fact that it should load in the browser.
