Instructions

You will create 5 entries for your Reflective Journal about a patient encounter. In the 5th entry, you will review the previous 4 entries and evaluate your progress in reflective practice over the course of the term.  Each journal should be a minimum of 400 words. 

The purpose of this reflective journal is self-reflection regarding the role in the process of self-reflection as a PMHNP provider. Through reflective practice, the student will evaluate their own emotional health and recognize one’s own feelings as well as one’s ability to monitor and manage those feelings. The point of the exercise is to learn yourself, your triggers, the types of cases you end up getting overly involved with, and those you’d rather refer to someone else. The idea is to be able to personally reflect on your behaviors/thoughts/decisions and how those impact you in the role of PMHNP. 

Address the following items:  

1. Describe your past experience in mental health or with someone with mental illness. 

· My experience is working with patients with substance use disorder and mental health illness to include :- schizophrenia, bipolar and depression.

2. What are the reasons you have chosen to work with this population?   

· I would also like to reduce the negative stigma associated with mental illness.

3. Discuss any concerns you have regarding this specific clinical course and population.

· My clinical site is at a community based outpatient clinic setting we see diverse population. Mental health conditions treated include anxiety/ depression, ADHA, bipolar, schizophrenia, and insomnia.

4. Identify personal and academic/professional goals for the clinical course and population.

· My goal is to work with underserved population suffering with mental illness to increase access to these population group. I would also like to reduce the negative stigma associated with mental illness.

Course Description

This course prepares students to assess, diagnose, and manage mental health care needs across the lifespan. Emphasis will be placed on underlying acute and chronic psychiatric/mental health diagnoses. Clinical opportunities will be utilized for all PMHNP to apply concepts in primary and acute care settings with adults and families

Describe your past experience in mental health or with someone with mental

illness.

I recently saw a patient who was bipolar with substance addiction in the nursing home

where I work as an APN. The patient was moved to the rehabilitation center to heal

from a recent fall that caused his ankle to become dislocated. The patient had a manic

episode the day after he arrived and started acting aggressively and requesting

oxycodone. The patient needed to be calmed down, and the staff and I had to tell him

that we were there to assist him in any way we could. A stat eval for psychiatrist who

was on site that day was contacted. The patient's medication was administered by the

psych NP. He received more food from the kitchen staff, and we set up a television in

the patient's room. The patient cooperated with staff during his stay at the institution

for the following week and was compliant with taking his medication as directed.

What are the reasons you have chosen to work with this population?

I have a soft spot for the geriatric population suffering from mental health issues. I've

witnessed many cases of misdiagnosis, wrongly administered psychiatric medicine,

and inadequate monitoring. Working with a specific group on mental health issues

can be quite gratifying. It can significantly impact the lives of persons dealing with

mental health concerns. It can also be used to learn more about mental health and

develop a better understanding of mental health individuals' difficulties. Working with

a specific population on mental health issues, on the other hand, can be difficult. It

can be challenging to witness patients suffering from mental health try to cope with

their situation.

Discuss any concerns you have regarding this specific clinical course and

population Adult/Geriatric.

According to the CDC, 20% of adults who are 55 years of age or older have a mental

health issue. Anxiety, significant cognitive impairment, and mood disorders are among

the most prevalent ailments (such as depression or bipolar disorder). Suicide instances

frequently involve mental health problems as a contributing factor. Common worries

include insufficient insurance coverage, the stigma associated with mental illness, denial,

and a lack of mobility.

The most crucial factors in developing and putting into practice interventions that may

assist in addressing the mental health needs of the adult and elderly population are

encouraging community involvement, demonstrating respect for cultural differences,

encouraging social interaction, and disseminating information about where people can

turn for help with mental health issues (Joshi et al., 2017).

Furthermore, the neighborhood may have a scarcity of mental health resources. This can

make it difficult for adults/geriatric population to acquire the assistance they require.

Elderly adult mental health is a dire issue that must be addressed. The mental health

course should adequately address the adult/geriatric population's needs. The system

should teach students how to spot the signs of mental health issues in adult and elderly.

Furthermore, the training should provide information on locating resources and services

for this population.

 To gain more knowledge about the mental health conditions that might impact

adult and geriatric population. This involves learning about the signs, causes, and

treatments of mental health illnesses.

 To gain knowledge of evidence-based therapies for mental health issues affecting

adult and geriatric population. This involves learning about various types of

treatment, medication, and other interventions that can help adult/geriatric with

mental health concerns.

 To acquire knowledge and abilities in dealing with adults with mental

health/substance abuse issues. This includes discovering how to establish rapport,

conduct therapy, and handle challenging behaviors.

 To understand the risk factors for mental health problems in adult/geriatric

population. Understanding the genetic, environmental, and psychological risk

factors for mental health disorders

 To gain knowledge on encouraging adult/geriatric population with mental health

issues on how to seek help when they see certain signs of their or loved one’s

mental health deteriorating. This entails understanding various prevention

measures, including early intervention and community outreach.
