What are those potential outcomes and how could they be measured?

The paper is to be written in APA (7th ed.) format, including a title page, appropriate use of headings to separate the main sections of the body of the paper, proper in-text citations, and a references page. 

The draft needs to  include, but is not limited to, the following elements:

· The purpose of the project, including providing the Aim Statement

· What measure(s) will you use to evaluate the proposed project? Reflect on the types of measures that were reviewed in your week 5 learning resources (structure, process, outcome).

Implementing the multidisciplinary care team approach will lead to improved patient outcomes and satisfaction, as patients' behavioral health needs will be addressed more comprehensively.

· The nursing staff will collect patient satisfaction scores.

· The incident reports will be reviewed by the nursing manager.

Data will be collected electronically and reviewed weekly by the project lead and team. The data will be displayed graphically to track trends and changes in patient outcomes and satisfaction. Decisions will be made based on the data to adjust the care team approach if necessary.
