prospectus outline

I attached about 5 different assignments that need to be follow in the format of the outline chart im attaching. you can use all my info on the assignments but into 3 pages only.  I need like 10-15 sources all of its in the assignments. 

I have to submit this to my advisors please do detail Doctoral work and fill out the chart as well.
