Landmark report
In the landmark report by the Institute of Medicine (IOM) in 1999 To Err is Human: Building a Safer Health System, it highlighted the need for a national approach in the prevention of errors in patient safety. Medication errors were identified as a significant patient safety concern.
For this assignment, read the American Society of Health System Pharmacists (ASHP) Guidelines on Preventing Medication Errors in Hospitals.
In 2-3 paragraphs, discuss interventions that are used in your current or previous practices to prevent medication errors. Include in this discussion which interventions do you believe are highly effective and others that although in theory are highly effective are more challenging. For example, administering medication with no distractions- although this is very important at times this is very difficult to achieve on a busy unit.
