Journal_Week_2.docx

Finding your "spirit of inquiry" this week, reflect on your current practice to identify a gap/problem (a quality improvement) opportunity you would like to address. When brainstorming, review the Agency for Healthcare Research and Quality's (AHRQ) Quality Indicators as a potential area of focus for your DNP Improvement Project.

The Population/Patient/Problem, Intervention/Interest, Comparison, Outcome, Time (PICOT) question is a framework to help outline your research question and guide you through the decision-making process when applying evidence-based practice changes. Developing a specific question with independent and dependent variables will assist as you dive into the literature review process. The keywords in your question will become the keywords to help you navigate finding that needle in the haystack. Reviewing the literature is the foundation and the first step to supporting your project. Diving into the evidence will further develop your knowledge about the problem, what practices have been created to address the problem, and aid in constructing the project's aim.

As your journey continues through the literature review process, the next step in your project development will be linking the project to a guiding framework for implementation. There are several Conceptual Frameworks to help guide project implementations. Your focus will be on selecting an appropriate Translational Science Model/Theory for the project as your framework. The Translational Science Model/Theory helps bridge the gap between Evidence-Based Practice (EBP) recommendations to practice change(s). The assignment this week will focus on the beginning steps of DNP Project Chapter Two development.

Approved Translational Models:

· Promoting Action on Research Implementation in Health Services (PARIHS)/i-PARIHS

· Knowledge to Action (KTA) Framework

· RE-AIM

· Iowa Model of EBP to Improve the Quality of Care

Now that you have identified a problem and are beginning the brainstorming process to address the change to improve the quality of care, which of the approved translational science models could act as the foundational framework for your project? Describe how you see this model applying to your project.

Explain how that theory assists in transforming the research in guiding evidence-based quality improvement change. Describe the translation science model you will be using or are considering using as a part of your project.

Journal Response Expectations

Each short-answer Journal response will be a minimum of 300 words.

No citation, reference, or APA style is necessary.

This is my Topic

“ This quality improvement project is to implement the use of an evidence-based self-care tool, Summary of Diabetes Self-Care Activities (SDSCA), to improve diabetes self-care behaviors among adults with type 2 diabetes.”

Targeted PubMed® Search

1. Register for the National Center for Biotechnology Information (NCBI) Account - (Click “sign in to NCBI” and follow instructions to create an account using one of the 3rd party sign-in options. Please make a note of your log in username and password).

2. Complete PubMed® activities by following these instructions:

· Identify a problem or area of potential quality improvement for your DNP Project.

· Using PubMed®, create a search strategy for your selected topic.

· Use MeSH® terms and Boolean operators (i.e., AND, OR, NOT).

After you complete each step below, record the number of references returned by your search. Include the frequency of "ALL" and "Review" for each step. Adjust your search strategy so that you can complete all steps.

1. Run a basic text search using two MeSH® terms for your topic.

2. Refine your search by using the LIMIT feature to include only references that are within five years.

3. Refine further to include references in which the search terms appear in the Abstract/Title.

4. Refine further still to include references in which the search terms appear only in the Title.

Use a tracking log to help keep track of your search results.
