Field Assignment, Part 1: 

Questionnaire

This is step one of the field assignment, and it involves drafting a questionnaire with at least two entry-phase questions and two contracting-phase questions. The following list contains examples of these types of questions. You may use the questions listed below, or you can choose to draft your own set of questions (as long as your questionnaire contains at least two entry-phase questions and two contracting-phase questions).

Also, feel free to insert additional questions to the questionnaire. 

1.Entry Question:What advice can you share with regards to building relationships and trust?

2.Entry Question: If I knew then what I know now about the entry phase, what would that be?

3.Contracting Question: What tools do you use to prepare contracts? At what point do you introduce a contract for the client to sign?

4.Contracting Question:If I knew then what I know now about the contracting phase, what would that be? The purpose of this assignment is for you to collect insight about entry-phase and contracting-phase strategies for the OD process while also actually using a data-gathering technique. 

You are only submitting the questionnaire for the Unit III Homework Assignment (no responses are needed for this assignment). This questionnaire will be used toward completing the Unit VIII PowerPoint Presentation, and that is when ORG 6800, Organization Development 3

you will submit the responses to the questionnaire. Time will pass quickly, so begin contacting the consultant about the questionnaire as soon as possible
