Possible Practicum Site Needs Assessment Interview Guide Form

Directions

You will use this interview form to guide an appropriate nurse leader of a possible practicum site. It will serve as an organizational needs assessment and guide you to discuss potential policy issues in a future class. Make sure to keep a copy of this form for future use.

Who: Name of the DNP student.

Whom: Name of the interviewee.

What: Describe the interviewee's preparation, education, and credentials (the requirement is that the professional to be interviewed must at the minimum meet the needs to serve as a potential preceptor for the DNP student's upcoming practicum experience). The criterion is that the preceptor must be a leader/ decision-maker, preferably doctor-prepared in nursing, and preferably (but not required) nationally-board-certified in the direct or indirect care area the student is pursuing (either an advanced practice nurse—CNP, CNM, CNS, or CRNA; or in nursing administration, nursing informatics, or nursing education).

Organizational Assessment:

1. What are some problematic areas of the practicum site?

The student elaborates on any possible policy that you must consider regarding the topic at hand. Is there any policy that contradicts or will prevent implementing a practice change? Is there any outdated policy and contributing to the issue or clinical? Is there any policy that you must respect in the process? Any policy regarding having to ask site permission to implement a practice change?
