SUBJECT: Volunteer Project Participation for UWA Student in CO 506

Life-Span Development and Learning

This term, students in CO 506, Life-Span Development and Learning at the University of West Alabama are asked to do an activity of practical experience with a person concerning a life challenge and building back resilience after the challenge. This will consist of the UWA student asking some questions of their volunteer. If the volunteer is 18 years old or younger, the UWA may also ask questions of a parent or caregiver and signatures would be needed for both the volunteer and the caregiver. The student will share the questions with you before getting your signature on this letter and give you time to think about the questions before officially noting your answers. Feel free to ask for clarification and also to share what you want to with the questions asked. Through this, the student will learn how to apply what he or she is learning in the class to help him or her work with people of all age ranges through many types of life challenges.

In order for the student to work with you or your child, we need the bottom of this form signed with both the volunteer and the parent or caregiver signature to indicate agreement to participate. As this is a class project, the student is asked to maintain strict confidentiality in the written information kept to submit to the instructor. This form of agreement is the only place where we ask for full name as you provide signatures.

I have signed this letter as the instructor and have included my contact information. Please contact me if you have any questions about this class project.

Thank you so much for working with the UWA student in provision of a valuable experience for the class.

Sincerely,

CHALLENGE AND RESILIENCE PROJECT 4

Summary and Reflection –Challenges and Resilience Project

Student Name

University of West Alabama

Narration for PPT Slide

In this section type your narration for your PPT slide on your volunteer’s challenge and resilience. This narration should be 225 to 300 characters of text that shares the essence of the person, challenge, and resilience built through meeting that challenge. (This length of narration will facilitate combining student slides into a presentation from the class on life challenges and resilience).

Summary of Interview

In this section provide a paragraph that describes the volunteer in demographic terms and then the volunteer’s answers to the # 2 interview question.

Immediate Reaction to Experience – Summarize the volunteer’s answers to the # 3 interview question.

Current Reaction to Experience - Summarize the volunteer’s answers to the # 4 interview question.

Negative Life Factors Influence - Summarize the volunteer’s answers to the # 5 and # 6 interview questions.

Positive Life Factors Influence - Summarize the volunteer’s answers to the # 7 and # 8 interview questions.

Current Personal Strengths - Summarize the volunteer’s answers to the # 9 interview question.

Advice from Volunteer - Summarize the volunteer’s answers to the # 10 interview question.

Pathways for Exploration

Begin this section with a paragraph of introduction on pathways that could help a person with to build resilience for this type of challenge.

Pathway of Prevention

In this subsection provide a one – page (250 – 300 words) discussion of a life factor that could be present in a person’s life before such a challenge is experienced that would provide some sense of immunity from impact. You should support this discussion with integration of at least one scholarly article reference (the best source for scholarly articles is the UWA Library with its databases). Examples would be factors that you and classmates discussed in the first discussion forum as “the most helpful influence in my life development.”

Pathway of Recovery

In this subsection provide a one – page (250 – 300 words) discussion of a life factor that could be present in a person’s life after such a challenge is experienced that would provide some sense of support and help in recovery. You should support this discussion with integration of at least one scholarly article reference (the best source for scholarly articles is the UWA Library with its databases). Examples would be again be those factors that you and classmates discussed in the first discussion forum as “the most helpful influence in my life development.”

References

On this page list any references that you cited in the paper above to support your discussion. APA style does indicate that references listed on this page should be cited in the text of a paper – any reference cited in the paper should be listed here and any reference listed here should be cited in the paper. Students should get correct formatting for this by referring to either the 6th edition of the APA manual (listed on the syllabus under Recommended Resources) or the Purdue OWL-APA Writing website at https://owl.purdue.edu/owl/research_and_citation/apa_style/apa_formatting_and_style_guide/general_format.html
