Fill out the Internship Learning Agreement form with your Internship Site Supervisor. Use the "How to Write Objectives" document as a reference when filling out the form.

As  you are filling out your objectives, please incorporate one of the  'Seven Areas of Responsibility' as outlined by the National Commission  for Health Education Counseling (NCHEC) into each objective and tell how  you will meet the objective.  Please remember that you need one  objective for each credit you are enrolled in. These seven areas are:

Area I:  Assess Needs, Assets and Capacity for Health Education

Area II: Plan Health Education

Area III: Implement Health Education

Area IV: Conduct Evaluation and Research Related to Health Education

Area V: Administer and Manage Health Education

Area VI: Serve as a Health Education Resource Person

Area VII: Communicate and Advocate for Health and Health Education
