HUS4321 Advanced Service Plan Project
For this project, you will create a Human Services organization and client to practice the skills learned in class.
The goal is to take your made-up client through the process of case management in your organization from
intake to discharge. There are two parts to your project. Part 1 is your Advanced Service Plan, and Part 2 is your
Slide Presentation.
• Part 1: The Advanced Service Plan for your “client”.
✓ There are three (3) steps to your Service Plan. They are built on each other and will be submitted in Assignments 9, 12, and 14.
✓ Please use the Advanced Service Plan Example and instructions below as your guide.

➢ Create your Human Services Organization.
1. Create a “Mission” Statement for your organization.
2. Outline the “Population Served.”
3. Outline the “Eligibility Requirements”.
➢ Choose a made-up client to be your practice “client” to demonstrate the skills learned in class.
4. Create your client profile “Client Information”.
5. Create your “Informed Consent and Confidentiality Procedure.” Make sure all limits of confidentiality are included.
6. Conduct an intake interview with your client using the skills learned in class. Create an “Intake Summary.”
7. Add your client’s “Strengths Assessment.” Assess needs and look for the client’s strengths. Be sure to include personal and environmental strengths.
8. Create your client’s “Diagnostic Summary Statement” (Needs Summary).
o Step 2: Assignment 12 upload
➢ Continue your service planning for your client. Add the below information to your Assignment 9 (continue where you left off). If you did not do Assignment 9, you will need to include the missing Assignment 9 information in this assignment to receive full points for Assignment 12 (50 points will be deducted if that assignment is not included).
➢ Create a plan for services using the skills learned in class.
1. Create your client’s “Treatment Recommendations” (Service Recommendations).
2. Create “Goals and Objectives”. You must have three (3) measurable objectives and three (3) outcome-oriented goals. The goals and objectives must be aligned with the client’s presenting problems.

3. Include at least five (5) “Community Resources” (referrals within the community).
4. Create at least three (3) “Progress Notes” using the SOAP format.
