Project title: Medication Errors in healthcare Centers in Saudi Arabian

Medical errors represent a serious public health problem and pose a threat to patient safety. All patients are potentially vulnerable, therefore medical errors are costly from a human and economic. medication errors were the most common type of medical error in all health care organizations. Medication errors are a significant global concern and can cause serious medical consequences for patients. Little is known about medication errors in Middle Eastern countries..

The main aim of study: assessment of medication errors in primary care centers in Saudi Arabia Objectives:

- To determine the baseline level of medication errors and then identify the nature of

medication errors

- To identify potential risk factors of medication errors and then patients at high risk of

medication errors.

- To identify factors contributing to medication errors and then suggest strategies that can

help to reduce /prevent medication errors and improve patients safety the quality of

health.

· To carry out an economic study to measure the cost for preventing medication errors and for re-admission to hospitals and then to carry out appropriate health economic analysis to measure benefits.

Study Area/Setting:

The study will be conducted at 3 public healthcare centres located in Saudi Arabia. Target : adult patients with chronic conditions (> 18 years old) in both gender

Data and Method :

In this project will employ quantitative and qualitative research methods

1) Direct observations and analysis of prescriptions.

2) Survey interviews and questionnaires will be conducted with healthcare provider (

 Nurses, pharmacists, physicians and community services).

3) Data Management and Analysis Plan will be by using by using Statistical Package for the Social Sciences (SPSS).
