My clinicals will be doing telehealth for hospice patients. I haven't started yet, but I think one important topic to assess for these type of patients are to focus a little in their families and their mental health. Families for hospice patients, specially the ones who are home taking full care of them, suffer so much and sometimes they do it in silence. So I think its important to also as nurses, pay attention to these families and how they are, emotionally and mentally. We should be able to provide resources for them within the community. So the hospice nurse together with the public health nurses should pay close attention to the family members and work together as a team to provide all the services available to them and maybe get like a little break from the house. A good topic will be how hospice care affect family members and ways or resources to help them a little. Relieving that pressure they feel when they are 24/7 taking care of the dying family member. It can be devastating and a little break for a day at least once a week, will be very beneficial for their mental health. One day of self care and relaxation to keep moving forward with the hospice process.

My topic proposal is bereavement. Currently, my clinical work is as a volunteer with VITAS. I call and provide support to primary caregivers and patients in hospice. Interpersonal communication skills and over the phone assessments are a critical part of my role. The ensuing allows me to speak to the VITAS nurse for aid with client referrals for additional support. For instance, if a PCG is demonstrating feeling overwhelmed with care for their loved one. I can alert their nurse to aid PCG in finding relief or assistance with respect. The calls are performed weekly; therefore, patience and proper communication are needed to build a rapport. Building a rapport allows for clients to be open and helps with grief. Grief can occur at different times and does not have a timeline. (Mental Health America, 2020). Knowing performing BRAT(The Bereavement Risk Assessment Tool) can help identify individuals who require additional support and treatment. (GRPCC, 2016).
