For the introduction, you are going to introduce your paper. Discuss the phenomena of the issue, and why there is a need for the change project. Give a brief overview of what this rough draft will include: a PICOT question, a literature review, project aims, values, and desired outcomes, conceptual and/or theoretical framework, interventions, and finally recommendations.

The introduction may take up to a page to fully discuss each part of the paper.

Proposed Project and PICO(T) Question

Problem Identification and Description

Describes the problem thoroughly, including the target population and history or background information of the problem at the current setting.

Describe the clinical setting, or the service area for the population. Is it a hospital setting, a clinic, outpatient, etc. Is this problem specific to this setting or is it a system-wide problem.

What is the significance of this problem and how is it applicable to nursing? What are some of the potential outcomes of this project?

PICO(T) Question

Give a clear and explicit statement of the problem, written in PICO(T) format. For example: In patients with head and neck cancer undergoing radiation therapy, how does magic mouthwash compared to viscous lidocaine affect pain from mucositis within two weeks of symptom onset?

Significance of Evidence-Based Project

Describe the significance of this project from a nursing perspective. If the problem relates to a patient population, describe the significance of the current practice and the outcomes related to it, and how this change project may change the outcomes for the better.

Discuss current (up to date) research on this topic, and note any current knowledge gaps. For example, using the PICO(T) question above, you could note that current treatment of radiation induced mucositis is initially treated with the topical anesthetic of 2% viscous lidocaine and benzocaine (Singh & Bagewadi, 2022). However, magic mouthwash is a combination of a topical anesthetic, antacid, and diphenhydramine that has both an anti-inflammatory and antimicrobial effect (Singh & Bagewadi, 2022).

Make sure that your research on the topic is up-to-date and try to use references that are within the last 2-3 years, but no more than 5 years old.

Literature Review

In this section, you will discuss each article, evidence-based guideline, standardized procedure, etc. There needs to be a minimum of 7 peer-reviewed or scholarly articles within the past 5-7 years.

You do not need to give me the title of the article, but you do need to note the authors. For example, you might note: In the study by Smith & Frank (2023)…….

Explain what the purpose of the study was for, and what were the results.

Each study needs to be its own paragraph and should have in-text citations to support the material noted.

Summary of Literature Review

In this section you are going to briefly explain how each study is relevant to your change project. Was any of the information new to you? Again, make sure that each article has its own paragraph, and cite as appropriate.

Knowledge Gaps

Describe any knowledge gaps that you found, and the effects they may have on advanced nursing practice roles as it relates to your project. If there was not a knowledge gap for a study, do not include it in this section.

Project Aims, Values, and Desired Outcomes

Give a brief overview of the goals, values, and desired outcomes of the project .

Project Aims

In this section, inform the reader of the purpose of this change project. What have you noted needs to change (based on your PICO(T) question. What is your proposed solution to the project. Try to explain the change process, and how this new process will generate new knowledge and outcomes.

Also, make sure you note the benefit of the change project in terms of time and cost. Will this project require a lot of man-hours to complete, and will it be expensive in the long run? How will this project affect those involved (patients, providers) vs. the healthcare institute. Make sure you are citing appropriately.

Values

For this section, discuss your goals and objectives. Remember to make them SMART goals in that they are measurable. Use the resources from last week, and see what their goals were, and how long it took to achieve them. Do you need to have approval of all stakeholders for this project (who are the stakeholders – anyone who will or may be affected by this project).

What variables are you working with, and do you have any control over them? If not, how will you work with them. For example, if you were to implement a clinic-wide practice change, would budget be an issue and how would you address that variable?

Lastly, how will you collect the data for this change. Again, look back at the studies you reviewed, and how did they collect data? Was it surveys? Was it incident reports, etc?

Desired Outcomes

Inform the reader of the intended outcomes of this change project. What are the current outcomes, and how will they change after this change project. Will the change be instant, or will it take some time. Is the project for the short-term, or is this a long-term project. Cite current studies and the results that they are noting.

Theoretical/Conceptual Framework

A theoretical framework is a general representation of relationships in your problem and is based on an existing theory. The conceptual framework is your idea on how you would like to explore your topic, and is based on a theory.

Remember that research never proves theory. Your research project can only support or refute the theoretical propositions you are using as your framework.

Identification of Framework

In this section of your change project paper, state the perspective through which the problem and/or phenomenon may be explored, and include the following:

Select at least two theories and discuss the process and logic for why this theory applies to your change project.

Assumptions of the Framework

What are the key components and assumptions of each of the theories or concepts for your framework. Describe these assumptions as they relate to your change project.

Element of Phenomenon

Apply each element of the theory or concept to the elements of phenomenon of the change project. Develop and explain the rationale for the sample selection criteria.

Discussion of Pertinent Research

Use this section as your conclusion to wrap up why you chose the theories/concepts to support your change project. Review and summarize what studies and articles you currently have that support the chosen frameworks.

Intervention for Proposed Clinical Change Project

Give an overview of what this section will cover: the need for intervention, the proposed interventions, comparison of other viewpoints, and the implications of your change project.

Proposed Solutions and Interventions

In this part of the paper, you are going to discuss why your chosen project needs interventions. Discuss your proposed interventions, and why. Do not reinvent the wheel! Go back to your literature review, and see what interventions other studies used, and cite those. If an intervention was not effective, note that in your paper. You could note: studies note that XYZ are good interventions in XYZ setting, but for this change project, they will not be utilized due to ABC.

Participant Selection Criteria/Sampling Population

For this change project, you have to decide what your population is. If there are people involved, what type of person are you looking for? For example, for a change project related to a change process on a M/S unit, the selection criteria would be all nurses on both shifts of the specific M/S unit, including and float nurses.

If it is related to a disease process, be specific. What diagnosis, what age group, do they have to be a specific weight, or on a specific medication. The more specific you are with your criteria, the more accurate your results.

Also, how large will your sample be? Are you looking for only 5 subjects, or can it be 100’s. Consider your timeframe as well.

Study Results

In this section, you are going to discuss how you intend to analyze your results. How will you collect the data, and use the data.

Limitations

Describe potential limitations. Are there any practical limitations that could affect your data collection? How will you attempt to control the limitations? What about the limitations of your sample? Will you have any potential issues getting people to participate?\

Gaps in Understanding

Review your interventions, and summarize what studies and articles you currently have that support the interventions. How will this new practice (intervention) fill the knowledge gap that you have found. If it was communication, will this new intervention/practice improve communication, and improve patient outcomes, patient satisfaction, etc.

Recommendations and Conclusions

This final section of your paper will summarize the project, its main points and findings, the significance of the project to the advanced practice nursing profession, and finally, the recommendations for future research.

Proposed Recommendations

For this section of the paper, you are going to want to discuss the recommendations that you are proposing for your change project. What are the implications of these recommendations for clinical practice.

Limitations of Study

For this section, go back into your literature review and see what other studies noted as limitations. Was it that there was not much data pertaining to the topic, was the participant pool biased. For the time frame, was it too short to collect data, or was the data collection too long.

Consider your goals and objectives for this study, and what concerns you may have had.

Summary of Research

Give a brief overview of the study, and what research supports the need for this change project. Provide suggestions and discussions that can direct your study for further research. Discuss if there is a need for a change to public policy, or is this more at the local/clinical level

Lastly, take into consideration any changes in theoretical constructs. Tie the theoretical or conceptual framework back to the study, and the results.

Conclusion

This is your final conclusion of your paper. This is your overall wrap-up of your change project. This should be a robust conclusion. Re-introduce your topic, and why there is a need for a change to happen. Give a brief review of the results – you might note, George et al, (2020) noted that …….. did xyz, which supports this change project. Again, note your suggestions for further follow-up or further research on this specific topic, and how this change will benefit the affected population.
