1. Brief introduction a) Topic/title of the presentation b) Explain why you chose to explore this particular health behavior.

2. Background on health behavior

a) Describe the negative health behavior and its current prevalence in the US. b) Who/which part of the population does the health behavior predominantly

affect? c) Using the ecological framework, describe the various determinants of the

health behavior in the population. d) What are some determinants of the health behavior that are specific to rural

populations? e) What are the short- and long-term effects of the behavior on health and/or

safety? f) Be sure to cite all of the scholarly sources you used to answer these

questions in the individual slides (this can be done using a superscripted number that corresponds to the reference list at the end of the presentation).

3. Application of two cross-cutting issues

a) Explain why you chose to explore these particular cross-cutting issues. b) How can these cross-cutting issues potentially be related to/impact the

process of changing your selected health behavior? c) Would the relationship of these cross-cutting issues with the selected health

behavior differ in a rural (versus urban or suburban) setting? If so, how so? d) Specifically, at which levels of the ecological framework do these cross-

cutting issues impact your selected behavior? e) Be sure to cite all of the scholarly sources you used to answer these

questions in the individual slides (this can be done using a superscripted number that corresponds to the reference list at the end of the presentation).

4. Program planning and evaluation a) Which planning model(s) did you use to develop your health behavior change

intervention? b) What is the target population for your intervention? c) Who are the key stakeholders for your intervention? d) What are your SMART objectives for the intervention? e) Describe your timeline for implementing your intervention. Do not forget to

include your evaluation strategies. f) What specific activities will be included in your intervention program? Also,

specify how the two cross-cutting issues will be addressed by these intervention activities.

g) Which types of evaluation (formative, process, outcome, or impact) would you conduct?

h) Describe your evaluation plan in terms of study design (experimental, quasi- experimental, or non-experimental).

i) Briefly describe how you plan to disseminate the results of your intervention.
