Topic 5 The Case Management Model Simulation Read the following case study and consider your approach when treating this client. Choose a model used in clinical social work and use the first three phases of the casework process to address the needs of this client and in 250-400 words total, address the prompts below. Cite your references in the “reference” section at the end. Jane is a 20-year-old woman who was referred to you by the health department at a local college where she is taking courses. It came to the attention of the school nurse that she has been engaging in risky behavior including excessive substance use and unprotected sex. Jane recently moved in with a friend after breaking up with her boyfriend. Her mother has been in and out of jail for the past 2 years on drug-related offenses and Jane has no relationship with her father. Currently, Jane works part-time at a local bar while taking classes but her grades have recently dropped and she is failing her classes. Model: Cognitive Behavior Therapy Study- From the perspective of your chosen model, what are the presenting problems in this case study? Cognitive behavioral therapy (CBT) is a psychotherapeutic model that can be used to help people
identify and understand the destructive thought patterns that have a negative impact on their
emotions and behaviors (Magill et al., 2019). It also seeks to help people identify comping
mechanisms that they can use to avoid the damaging or destructive patterns. It therefore focusses
on challenging and changing the negative thoughts that can contribute to destructive behaviors so
that it can replace them with more objective and productive thoughts.
From the perspective of the chosen model, there are two main presenting problems in the case
study that have the potential of resulting in other serious problems for Jane. First, there is a problem
of substance abuse resulting from Jane’s excessive substance use. Coupled by her part time job at
the bar, Jane is more exposed to substance abuse which may be part of the cause of her reduced
academic performance. Second, there is the problem of engaging in unprotected sex which may
make Jane susceptible to sexually transmitted infections and diseases.

Assessment- Determine the origin of the presenting problems. Indicate strategies that you would utilize to further assess the client's presenting problems (e.g., what questions could you ask to better determine the underlying cause or motivations of the presenting problems?) The origin of the substance abuse is Jane’s mother who has been in and out of jail for drug-related
offences. It is possible that Jane learned the behavior from her mother as stipulated by Bandura’s
social learning theory. The theory argues that people take up new behaviors by observing,
modelling and imitating the behaviors of others (Nabavi, 2012). Moreover, Jane’s part time job at
the bar may also contribute as the origin of the problem of substance abuse. The origin of Jane’s
risky sexual behavior stems from both the external environment and her mental state. Bandura
noted that external environment is not the only factor that influences learning and acquisition of

To further assess the presenting problems, I would initiate counselling and psychotherapy within
which I would ask more questions to understand Jane better (Efrati & Gola, 2018). For instance, I
would ask Jane about her extended family and whether or not she is in contact with them. Such
questions would help me understand the social nature of the presenting problems and the specific
interventions to consider.

Intervention- Use your chosen model to determine interventions that you could use to treat the client. Explain the merits of your chosen interventions. Based on Jane’s presenting problems, the aim of any intervention should be to help her stop using
substances and engaging in risky sexual behavior. First, I could identify the triggers that
predispose Jane to the two activities. This intervention would assist in identifying the negative
thoughts and in understanding how to eradicate the behaviors. (Cherry, 2021). Second, I could
work with Jane to set achievable goals so that she can focus on the process of recovery rather than
on the final result. This will ensure that Jane makes significant improvements in changing her
destructive behavior.
