1.Anxiety disorder
A 38 year old patient visited the health care facility due to constant anxiety over her family wellbeing. The patient who is a single mother reported that after lack of permanent employment has contributed to the increase her anxiousness. Additionally the patient also confirmed that the anxiety has increased to a point of affecting her sleep.
In today’s psychotherapy session the patient was engaged in approaches which would enable her to effectively manage her anxiety. The patient was enlightened on the best way to manage her time through following a daily schedule. Also, she was enlightened on the essentialness of quality time of sleep. The patient was advised to continue participating in psychotherapy sessions in order to effectively manage her condition
2. Mood disorder
A 30 year old patient visited the health care facility due to his manic depression which impact his normal life activities. The patient report over the past two weeks he has not been able engage with the people around him especially in his workplace and this has caused him to perform poorly.
In today’s psychotherapy session the patient was enlightened on the essentialness on managing his moods in order to be productive in the workplace. The patient was advised to frequently engage in physical exercises as they promote both mental as well as physical health. Additionally, the patient was also advised to avoid taking sugary foods, alcohol as well as caffeine.
3. Sleep Disorder
A 40 year old patient visited the health facility due to her inability to sleep due to issues related to her financial status. The patient visited the facility with her husband. She reported that after loosing her job she has been having difficulties sleeping and the irregular sleeping routines impact her ability to conduct her daily chores.
In the psychotherapy session the patient was enlightened on the how to effectively manage her condition. The patient was advised that it is essential for her to follow a particular sleeping routine in order for to manage her condition and be able to effectively engage in her daily activities.
4 Psychotic disorder
A 70 year old patient visited the health care facility due to his psychotic condition which is majorly caused by Alzheimer’s Disease. The patient visited the facility with his son. The son reported that the psychotic condition begun being identified after contracting the Alzheimer’s disease two years ago.
In today’s psychotherapy the patient and his son were being enlightened on various approach to manage and live with the psychotic condition. The son was advised to ensure he is not judgemental towards his father as well as ensure that he speaks slowly as well as simply. The son was advised to encourage his father’s self-confidence.
5. Mood Disorder
A 66 year old patient visited the health facility due to significant depression episodes attained after losing his wife through a road accident. The patient reported that his inability to effectively cope with his wife’s death has significantly impacted his ability to engage in his daily life activities. He also reported that after engaging in several physical activities he felt less sad and he embraced life.
In today’s psychotherapy session the patient was encouraged to develop a schedule to promote his as routine work out. Over the past one week the patient has been able to sleep and the mood disorder symptoms have been gradually reduced. The patient was encouraged to continue with psychotherapy sessions to effectively improve his condition.
6. Eating Disorder
A 14 year old patient visited the health care facility due to lack of eating due to the fear of being overweight. The patient was brought in by her father. He reported that the daughter had being over weight at the age of nine but after overcoming the health issue he has not being eating properly and now it has gotten worse.
In today’s psychotherapy session the patient was encouraged to externalize as well as diffuse her thoughts before following them as a command as a well as controlling her behaviour via support to make the patient to feel more secure. The patient reports that over the past one week she has been feeling positive about herself as well as the decision he makes when it comes to her eating condition.
7 Sleeping disorder
A 58-year old patient visited the health facility due to his inability to sleep effectively. The patient reported to having sleep less nights after his shift in the workplace was changed from day time to night shift. He asserted that after the shift alteration he has been having trouble sleeping at night when he is on day shift.
In today’s psychotherapy session the patient was being enlightened on the various activities which may enable the patient improve his sleeping condition. He was advised to develop a sleeping schedule when he is on day shift which would enable him to sleep.
The patient assert that over the past one week he has being engaging in physical exercises and they have contributed in the reduction of sleeping disorder symptoms. The patient was advised to continue with the psychotherapy sessions to improve the condition.
8 Mood Disorder
A 72 year old patient visited the health facility due to her manic depression which is caused by her extensive usage of substance abuse. The patient reported that her extensive usage of substance abuse has contributed to her mood disorder.
In today’s psychotherapy session the patient is being educated on the various approaches of reducing stress in order to improve the patient’s mood condition. The patient reported that she have been having severe bipolar conditions when she was faced with stressful situations. The patient was advised to continue with the psychotherapy session to effectively enable her to manage the condition.
Plan:
The patients were advised to continue with the outpatient psychotherapy.
