For this assignment, groups will decide on one EBP from the EBP assignment and present a voice
narrated Power Point on an effective evaluation plan to assess the success of the program and program
impact. This evaluation plan is being completed on the EBP, NOT on the CCNL.
Assignment Instructions:
You must have a 6-8 slide (not including title and reference slides) voice narrated PowerPoint.
Script should be in the presenter’s notes at the bottom of each slide. Each slide should have a voice
narration no longer than 2 minutes. I will stop grading at the 2-minute mark. If the presentation is too
large to upload to the assignment dropbox, use Filestogeaux and upload the link in the comments section.
Slides should consist of:
EBP
-General information about the EBP
-Name of the EBP
-What problem does the EBP aim to address?
Hypothesis
-Three questions the evaluation seeks to answer.
-Your hypothesis (If we do this___________, then that_________________ will happen).
Evaluation's purpose
-What type of evaluation of the EBP would you use? Why? (Ch. 8, Hoefer)
-What is the purpose of the evaluation?
Evaluation Findings
-How will you use the findings?
-Draw a logic model to depict (one input, one activity, one output, one outcome)
Anticipated Evaluation Results
-What will you know AFTER the evaluation that you did not know before?
-What will you do as a result of the evaluation that you couldn't do before because you lacked the relevant
information?
Community Impact
-How will the clients and community of CCNL be better because of the implementation of the EBP?
