Background: Obesity is a risk factor for many diseases, hospitalization and mortality.

Problem: Obesity is increasing in prevalence and may not be managed well.

Research question: Do primary care physicians document obesity and formulate a management plan?

Research method/design: observational study using existing data (Mayo primary care database)

Population, sample, participants:

Population: Obese adults (Olmsted Co., MN?)

Sample: Obese adults with a general medical exam at the Mayo Clinic between Nov 1, 2004 and Oct 31, 2005.

Participants: All subjects in the “sample” with complete data.

1. diagnosis of obesity, 2. treatment plan

Demographics (Age, sex, BMI), Physician characteristics, Comorbid conditions

Data analyses:

Multivariable logistic regression to examine relation between dependent and independent variables using odds ratios

Two regressions, one for each dependent variable (diagnosis of obesity and documentation of a treatment plan)

Results

Only 19.9% had a diagnosis of obesity

Only 22.6% had an obesity management plan

The diagnosis of obesity was less likely (OR < 1.0) among men and older patients and more likely (OR > 1.0) among those with a BMI > 35, diabetes or sleep apnea.

The diagnosis of obesity was the strongest predictor of having a treatment plan (largest OR)

Discussion

A gap exists between physician practice and management of obesity.

Multiple barriers to identification of obesity exist.

Diagnosing obesity is the best predictor of developing a management plan.

Limitations

Obesity may have been considered without documentation.

Data came from a single institution.

The effect of the management plan on weight loss was NOT evaluated.

Effect of other variables (race, education, income) on the diagnosis of obesity could not be evaluated.
