OBJECTIVE

The final paper describes your personal change project, detailing the project, process, and key learning about implementing this change. In addition to a detailed description of your process, this paper needs to integrate ideas from the course that were most insightful for you in implementing this change. Before writing your final paper, make sure to review the grading comments on your initial project proposal.

1. My personal change: A brief description and explanation of your personal change goal. (½ page) Please review your original change project proposal.

2. Initial versus desired state versus current state: A brief inventory of the initial state (when you started), the desired state (what your goal was) and current state (where you are now at the end of the project) in measurable terms using the format below. (½ page)

For example:

Initial state

Desired sate

Current state

· I meet 40% of deadlines

· I use a planner for only 30% of my classes and work schedule

· I plan 30% of my assignments ahead

· 100% of my assignments are in my calendar at the beginning of the semester

· I meet 75% of deadlines

· I use a planner 4 days/wk

· I plan 50% of assignments ahead

3. Implementing my action plan: To what extend did you execute the action plan you developed at the beginning of the semester? What did you do, what actions did you take or what behavior did you change to help you to get closer to your desired state? (1 page)

4. Highlights and challenges: What is the one thing you are most proud of and why? What was the most challenging for you in implementing the change and why? And what did you do to solve this? (½ page)

5. Support systems: What support systems did you develop and/or use and in what way did they help you or not? (½ page)

In what way did the concepts and theories discussed in this course applied to your personal change process? Describe at least three and be very specific in describing how the concepts or model applied to your personal change process. Note: If you apply Kotter’s Model that counts as one concept or theory. (1½ page)
