Clients Presentation:

Mr. X (not his real name) is a male of Asian origin. He is aged 75 years old and over the last few months, he has been experiencing respiratory problems. He rarely visits clinics for medical checkups until recently when he felt that things are not right due to difficulties associated with breathing especially when carrying out heavy duties or walking up the stairs. The patient confirms to have smoked since he was 20 years old up to date. He also laments of productive coughing along with dyspnea on exertion (American Lung Association, 2020).

Subjective Data (4 points):

The client complains of experiencing a wide range of challenges. Some of the subjective data gathered from the patient included.

· Difficulty in breathing more so when engaged in vigorous exercises.

· Increased rate of breathing beyond the normal one.

· Produces granting sound during exhalation.

· The patient produces a wheezing sound when breathing.

· Profuse sweating on the head but with cold and clammy skin when felt

Objective Data (4 points):

The objective assessment of the patient aims at interpreting the sot crucial signs of the respiratory presented by the patient. With the regard to Mr. X the objective assessment data will include;

· Checking the rate of respiration.

· Looking for any abnormalities by assessing the shape of the client’s chest.

· Gathering data on the labored breathing challenges faced by the patient.

· Assessing the condition of thorax (American Lung Association, 2020).

· Conducting oximetry test.

· Carrying out blood tests to ascertain whether the patient is suffering from pneumonic-related issues that predispose him to respiratory problems.

Describe 2 Actual/Potential Risk Factors (2 points):

Potential risk factors for respiratory problems and associated diseases include;

Tobacco smoking: increase smoking and exposure to secondhand smoking predispose one to the risk of contracting respiratory diseases due to progressive damage of the integrity of the respiratory tract Chen et al., 2019). Smoke normally damages airways thus increasing chances of developing respiratory infections.

Age: aging is directly associated with respiratory problems because it increases the susceptibility to a wide range of chronic illnesses. Aging also affects effectiveness in coughing which clears respiratory airways and this means aging often results in declined lung functioning (Sullivan, 2012).
