Case Study:
Your new client Sam is a 26-year-old male identifying individual. Sam's gender pronouns are he/him. Sam does not want to be identified as transgender. He states "I am male, that's it". Sam was assigned female at birth (AFAB). His expression is male and he has been binding ( https://transguys.com/features/chest-binding (Links to an external site.) ) for the past 3 years. Sam reports that he is interested in pursuing hormone replacement therapy (HRT) and potentially "reassignment surgery" and would like more information. He is worried that he will have to be diagnosed "with some kind of mental disorder" in order to receive services.
Sam's birth name is Emily and he would like more information on how to legally change his name. He will also need information on the process of requesting a gender marker change https://transgenderlawcenter.org/resources/id/state-by-state-overview-changing-gender-markers-on-birth-certificates and name change on his birth certificate and other documents.
Sam recently married his long-term partner Lisa (they have been together for three years) and they would eventually like to have children. Sam reports he has a very loving family and his parents have "always supported him". Sam reports that Lisa's mother refers to him as "that thing" and has encouraged Lisa to leave him.
He is a non-smoker, non-drinker and has never used illegal substances.
He reports that he has experienced what he terms "panic attacks" during the last 6 months. When this is happening he experiences increased heart rate, feels sweaty, dizzy, and short of breath. Assignment Questions:
1) Identify two (2) areas of knowledge, awareness, and skills you would need in order to work with Sam.
2) What questions/areas of information are particularly important to focus on when conducting your biopsychosocial assessment of Sam?
3) Sam is considering hormone replacement therapy, what issues about body changes, sexual function, and other concerns should you discuss?
4a) Summarize transgender affirmative cognitive behavioral therapy.
4b) Identify two (2) ways you would incorporate transgender affirmative cognitive behavioral therapy in your work with Sam.
5) Respond to at least one (1) classmate's posting.
