Because of my borderline elevated HgbA1c and low HDL, I will keep a food diary for two weeks and return to the clinic to discuss my lifestyle habits with a registered dietitian.

Strong family support for making lifestyle changes

Education in nutrition from undergraduate

Receives WIC/SNAP benefits (food vouchers and farmer’s market coupons)

 

Lives in a neighborhood safe for walking

Barriers:

No transportation; must ask for rides

Residence is in a rural food desert; only foods available within walking distance are from a convenience store with limited produce (except when produce is available from the farmer’s market)

Low income; difficulty affording regular trips for gas, groceries, and gym costs

· Reflect upon the differences between your patient and your SDOH facilitators and barriers. Where are your blind spots and what are your biases in this case? What role do your particular “privileges” have in this situation?

· How would you approach making recommendations with this patient, given their SDOH facilitators and barriers? Consider upstream, midstream, and downstream strategies – list at least two for each category.
